Is coffee really addictive?

Caffeine is not considered an addictive substance in the strict sense—the DSM-5 doesn’t classify it as a substance use disorder. But caffeine withdrawal is recognized: headaches, fatigue, and irritability upon sudden cessation for 1–3 days.

Why that is

The question sounds simple, but the answer depends on how you define “addiction.”

What happens in the body:

Caffeine blocks adenosine receptors in the brain. Adenosine signals tiredness—when caffeine occupies the receptors, you stay more alert. With regular consumption, the body creates more adenosine receptors to compensate for the caffeine effect. That is building tolerance: you need more caffeine for the same level of alertness.

Withdrawal symptoms occur when the body suddenly gets no more caffeine:

  • Headaches (most common symptom, often severe)
  • Fatigue and lethargy
  • Irritability and bad mood
  • Concentration problems
  • Onset: 12–24 hours after the last caffeine intake
  • Duration: 1–3 days, rarely longer than a week

Why it’s still not an “addiction”:

The DSM-5 (Diagnostic and Statistical Manual of Mental Disorders) defines addiction by loss of control, social impairment, and continued use despite harm. Caffeine generally does not meet these criteria. There’s no extreme escalation of dosage, no loss of control, and no severe social consequences. Compared to nicotine, alcohol, or other substances, caffeine is harmless.

Tip for cutting back: Don’t stop abruptly; instead, reduce gradually over 1–2 weeks. This helps you avoid withdrawal headaches almost entirely.

At Sophienstraße 27, I tell guests who are worried: coffee is a habit, not an addiction. If you want to drink less, you can slowly cut back—or switch to decaffeinated coffee. It tastes much better today than it did ten years ago.
